Student Registration (Enrollment)

You will need to bring your student’s immunization record, certified birth certificate, and proof you reside within Camp Verde Unified School District’s boundaries. This proof could include a phone, electric or gas bill that verifies your physical address.

The student must be age five (5) by September 1, 2010 in order to register for Kindergarten.. (Please see above paragraph for required documents.)

If you need a certified birth certificate for your student, please use the following address to access the Arizona Department of Health Services. http://www.azdhs.gov/vitalrcd/index.htm
